This is a senior individual contributor role within a small, collaborative team. You are the primary delivery engine for day-to-day change management work across the program, reporting to the Change Management SME for daily priorities; rolling up to the LTC program Director.

Role Description

This is a hands-on, flexible change management practitioner role. You will help drive LTCT change execution across multiple workstreams by flexing between readiness and adoption assessment, change analysis, communications support, and learning and development enablement as program needs shift. Because the team remains small and lean, success requires strong ownership, sound judgment, and willingness to move between strategic support and detailed execution without waiting for rigid role boundaries. In this role, you will:

Support the design, deployment, and analysis of stakeholder readiness assessments, adoption assessments, pulse checks, and other feedback mechanisms in collaboration with the Sr Change Management PM.

Translate change data, resistance patterns, and readiness findings into executive-ready insights that inform leadership decision-making.

Build, activate, and sustain a change champion network, including onboarding, operating cadence, feedback loops, and engagement materials.

Deploy resistance observation tools through the network to surface emerging patterns and system workarounds early, with recommended responses.

Design and execute function-level behavioral change strategies that shift how people work, not just awareness campaigns.

Apply audience segmentation, behavioral nudge strategies, and data-driven approaches to drive measurable adoption across affected employee populations.

Lead technology and system adoption using appropriate approaches and interventions depending on the technology and stated requirements.

Track behavioral and adoption indicators at the function level to identify needed engagement, support, and enablement, flagging early signals of adoption risk.

Support the creation, management, and tracking of program communications in partnership with the Change Management SME and other program partners.

Draft, edit, route, and maintain communications artifacts such as key messages, talking points, FAQs, stakeholder updates, change network materials, and cascade materials.

Maintain awareness of the communications calendar and flag gaps, conflicts, or sequencing risks to the Change Management SME and Program Director when needed.

Support learning and development activities, including needs analysis, training readiness, learner audience coordination, enablement materials, reinforcement assets, and post-training adoption feedback loops.

Partner with program, business, communications, learning, and workstream leads to coordinate change activities and ensure these efforts are connected.

Support KPI and status data collection across the change management workstream in support of Change Management SME reporting.

Operate as an individual contributor who can take direction, independently move work forward, and escalate risks with concise recommendations.

What Success Looks Like

Readiness and adoption risks are identified early, analyzed clearly, and translated into practical actions for impacted audiences.

The change network operates on a consistent rhythm and produces usable feedback without requiring daily management by the Change Management SME.

Communications are drafted, routed, tracked, and sequenced in a way that supports the broader cascade plan and minimizes audience confusion.

Learning and enablement needs are surfaced early and connected to readiness, adoption, communications, and reinforcement plans.

Resistance themes, workarounds, and misinformation are catalogued, understood, and addressed through appropriate mitigation actions.

Behavioral change and adoption indicators are visible at key program milestones, not limited to awareness or training completion

The Change Management SME and Program Director receive concise, forward-looking program intelligence from this role.

The role flexes across changing priorities while maintaining quality, urgency, and clean handoffs in a lean team environment.

What You Bring

10+ years in organizational change management with direct experience on enterprise-wide transformations.

Experience supporting enterprise-wide or cross-functional transformations, preferably involving technology, process, operating model, or customer-facing business change.

Ability to flex across change management activities, including readiness and adoption assessments, change analysis, communications, learning and development support, stakeholder engagement, and change network operations.

Change management certification is a plus but not required. Relevant experience may include Prosci/ADKAR, CCMP or equivalent change management methodologies and approaches.

Demonstrated ability to independently manage assigned workstreams or deliverables while staying aligned to Change Management SME direction and program priorities.

Strong analytical capability: you identify patterns in resistance and adoption data, not just report numbers.

Strong written communication skills, with the ability to convert complex transformation information into clear, audience-ready messages and materials.

Practical learning and enablement orientation, with ability to support training readiness, learner engagement, reinforcement, and adoption feedback loops.

Peer-level facilitation skills across mid-level and senior leaders who do not report to you.

Comfort operating in ambiguity on a program that will evolve over multiple years.

Experience in a SaaS, software, or technology business model transformation.
